Boarding Policy Agreement

We are pleased to welcome you to our practice. Please take a few minutes to read our boarding policy.

Mountain View Veterinary Health Center ( MVVHC ) is pleased to be able to offer a flexible boarding service to our clients, allowing ½ day increments according to the following policy:

  • The client agrees to clearly label with their last name any personal belongings brought in with their pet(s). MVVHC is not responsible for any lost and/or destroyed personal belongings and does not guarantee the return of such items.
  • To qualify for a ½ day on the first day of boarding, the client understands they must schedule their pet’s arrival time for 12:30 p.m. or later. If the arrival time is not specified when the reservation is made, or if the client arrives prior to 12:30 p.m., the client understands a full day of boarding will be scheduled and charged.
  • To qualify for a ½ day on the last day of boarding, the client understands they must schedule their pet’s discharge time for 12:30 p.m. or earlier. If a discharge time is not specified when the reservation is made, or if the client arrives later than 12:30 p.m. to pick up their pet(s), a full day of boarding will be scheduled and charged. The client also understands these deadlines are not flexible and will be strictly enforced. This is necessary to provide the kennel personnel sufficient time to thoroughly clean, sanitize, and prepare boarding accommodations for the next guest.
  • To ensure a healthy environment for our boarding pets, we require written documentation of an examination by a veterinarian within one year of the pet’s stay.

The following intestinal parasite control is required for all animal boarding at MVVHC. Official written documentation of veterinarian purchased or prescribed and administered medication prior to animals being admitted for services not performed by MVVHC. Boarding pets are required to have been dewormed with a full spectrum dewormer within one (1) year of their boarding stay:

  • Dogs: Must be praziquantel/pyrantel pamoate/febantel (Drontal Plus) or equivalent and indicated for the removal of tapeworms (Dipylidium caninum, Taenia pisiformis, Echinococcus granulosus and for removal and control of Echinococcus multilocularis), for removal of hookworms (Ancylostoma caninum, Uncinaria stenocephala), roundworms (Toxocara canis, Toxascaris leonina), and whipworms (Trichuris vulpis) in dogs.
  • Cats: Must be Pyrantel/praziquantel (Drontal) or equivalent and indicated for the removal of tapeworms (Dipylidium caninum, Taenia Taeniaeformis), hookworms (Ancylostoma tubaeforme), and large roundworms (Toxocara cati) in cats and kittens.

    The pet must have a negative ova and parasite fecal analysis and negative fecal hookworm, roundworm, and whipworm antigens by ELISA (all performed by a veterinary reference laboratory) no more than two weeks prior to the pet’s stay.

The following vaccinations are required for all animals boarding at MVVHC. Official written documentation of veterinarian-administered vaccinations, including their given date and expiration date, is required prior to animals being admitted for vaccinations not administered by MVVHC.

Dogs:

  • Bordetella (Intranasal or Oral given within six months)
  • Parvo-Distemper or appropriate titer level reports (according to MVVHC’s current vaccination protocol for the duration)
  • Rabies (according to Utah State Law)

Cats:

  • FVRCP- Distemper or appropriate titer level reports (given within one year and at least seven days prior to boarding)
  • Rabies (according to Utah State Law)

Client authorizes MVVHC to immunize their pet(s) with all required vaccines and/or intestinal parasite control, which are overdue or not documented in writing prior to being admitted to the boarding facility.

MVVHC provides a high-quality adult diet for boarding animals. Clients may bring special diet food for their pet. All pet food brought into the clinic must be in an airtight, re-sealable container. There is no discount for bringing your pet’s own food. In the event of insufficient food for your pet’s stay, we reserve the right to open and charge you for a new bag of food if a prescription diet is required or to feed your pet an appropriate wellness diet for the remainder of the stay.

MVVHC is happy to arrange for after-hours drop-off or discharge of your boarded pet for an additional fee. However, the availability of this service is dependent on the attending animal caretaker’s schedule. If the client opts for this service, fees for all boarding and other services must be paid in advance.

MVVHC staff will start the confirmation process two days prior to the arrival date of the reservation. Our staff will email and/or text a boarding check-in form that must be filled out prior to arrival. Any cancellations or no-shows that occur after the confirmation will incur a late cancellation fee equal to one day of boarding for each cage reserved. The client agrees to pay all such fees. Should the client fail to arrive on the beginning day of the reservation, the entire reservation will be canceled.

The undersigned shall indemnify MVVHC against any and all expenses, liabilities, and claims by or on behalf of any person or entity arising out of a failure to disclose a communicable or infectious disease, the spread of any disease, or any other injury occurring to the client’s pet(s) while being boarded.

The agreement will remain in effect from this date forward until such time as the MVVHC Officers change the policy.

We will gladly prepare a written estimate if you desire (please ask our client relations personnel or doctor). ALL PROFESSIONAL FEES ARE DUE AT THE TIME SERVICES ARE RENDERED. There will be the maximum lawful service charge for any check returned unpaid. Any balance over 30 days will receive a $5.00 monthly repeat billing charge and accrue interest at 1.75% per month (21% per year). Any balances referred to a collection agency will incur a 40% collection referral fee.

To prevent the spread of infectious diseases, all hospitalized/boarding patients must be current on all vaccines and free from internal and external parasites. The signature below authorizes this level of preventive care, and the appropriate charges will be assessed in the discharge invoice.
