Grooming Policy & Sedation Agreement Form

At Mountain View Veterinary Health Center (MVVHC), we are focused on providing a fear-free experience for your pet. Our staff members are trained to interpret body language and respond appropriately to your pet. However, in some cases, pets are still too fearful or anxious to allow us to groom them in a safe manner. When this occurs, we recommend administration of calming agents or sedatives to reduce patient anxiety and ensure that we can care for your pet without causing undue stress.

LEVEL 1 ($25+): Oral sedation will be administered, and your pet will be given time for the medication to take effect before we resume the grooming appointment. If this is effective, we may recommend that you take home a prescription sedative that you can administer prior to future grooming appointments.

LEVEL 2 ($160+): Injectable sedation will be administered, and your pet will be monitored by a qualified veterinary technician through the course of the grooming appointment and during recovery.

LEVEL 3 ($280+): Injectable sedation and anesthesia will be administered, and your pet will be monitored by a veterinarian through the course of the grooming appointment and during recovery.

Additional Terms

  • To prevent the spread of infectious diseases, all hospitalized/boarding patients must be current on all vaccines and free from internal and external parasites. All vaccines and/or intestinal parasite control requirements according to MVVHC are outlined in the vaccine and deworming policy.
  • The undersigned shall indemnify MVVHC against any and all expenses, liabilities, and claims by or on behalf of any person or entity arising out of a failure to disclose a communicable or infectious disease, the spread of any disease, or any other injury occurring to the client’s pet(s) while being groomed.
  • The agreement will remain in effect from this date forward, until such time as the MVVHC Officers change the policy, or you give written notice to change these authorizations.
